| Chyluria |
This August I found a sentence saying that chyluria cannot be cured in a WHO manual at the fourth annual workshop of filariasis elimination in the Pacific region. I will introduce our own experience on the treatment of chyluria through which we solved this issue about 30-50 years ago.
Dr.Katamine of Nagasaki University published in 1952 a series of papers (in Japanese, partially with English summary) which dealt with pathogenesis and a unique surgical treatment of chyluria. He surgically interrupted whole the lymphatic vessels connecting to the affected kidney in 5 chyluria cases. All these cases revealed successful results. Later his method was extensively developed by Drs. Okamoto and Ohi of Kagoshima University. They later summarized their result obtained by surgical stripping and interruption of renal pedicle lymphatics of affected patients during 1952 and 1973. The short-term results were excellent and chyluria ceased in 375 of 383 patients. However, 2 years or later 41 (25.6%) showed recurrence in 160 chyluria cases followed up. However, in comparison with renal pelvic instillation with silver nitrate solution, the surgical treatment revealed better result.
In any endemic areas with filarial chyluria, I recommend to take note our past practice mentioned here.
